A young man with exertional heat stroke is in a cold-water immersion tub with continuous rectal temperature monitoring. At which rectal temperature should he be removed from the water?

  1. A37.0 degrees C, once the temperature is normal
  2. B38.0 degrees C, once he stops shivering
  3. C39.0 degrees C, to avoid overshoot hypothermia
  4. D40.0 degrees C, once his confusion begins to improve

Answer: C. Cold-water immersion guidance for exertional heat stroke advises removing the patient at a rectal temperature of about 39 degrees C. Body temperature keeps falling after removal, so continuing to normal risks overshoot hypothermia, while stopping at 40 degrees C leaves him in the damaging range.
