A man with type 2 diabetes on basal-bolus insulin has decided, with his team, to fast. He normally injects rapid-acting insulin before breakfast, lunch and dinner. Which change to his rapid-acting insulin is consistent with IDF-DAR guidance?

  1. AOmit the lunchtime dose
  2. BGive the usual three doses, moving the lunchtime one to mid-afternoon
  3. CDouble the suhoor dose to cover the hours of fasting
  4. DHalve the iftar dose and keep the suhoor dose unchanged

Answer: A. For prandial (rapid-acting) insulin, IDF-DAR guidance is to take the normal dose at iftar, reduce the suhoor dose by 25-50%, and omit the lunchtime dose. Rapid-acting insulin covers a meal, and there is no midday meal while fasting.
