A housekeeper is pricked by a needle left in a bundle of used linen. The source patient cannot be identified. The housekeeper brings the needle to the emergency department in a container. What should the nurse do with the needle?

  1. ASend it to the laboratory for HIV and hepatitis testing of the blood inside
  2. BDiscard it safely in a sharps container, because the needle is not tested
  3. CKeep it in the fridge until the infection control team has decided
  4. DSend it for culture so that the results can guide antibiotic choice

Answer: B. Needles and other sharp instruments are not tested for HIV or hepatitis. When the source is unknown, the decision about post-exposure prophylaxis is made case by case on the severity of the exposure and how likely it is that the source was infected; the exposed person is assessed and followed up.
