A 66-year-old man presents to the emergency room reporting chest pain. He reports taking tadalafil for erectile dysfunction about 30 hours ago. An order for sublingual nitroglycerin is written. What should the nurse do?

  1. AGive the nitroglycerin, since more than 24 hours has passed since the tadalafil
  2. BGive the nitroglycerin and lay the client flat to offset a fall in pressure
  3. CHold the nitroglycerin and notify the provider that tadalafil was taken within the past 48 hours
  4. DGive half the dose of nitroglycerin to lower the risk

Answer: C. Phosphodiesterase-5 inhibitors potentiate nitrate vasodilation, causing severe hypotension and possible cardiac ischemia. Nitrates are avoided for at least 24 hours after sildenafil or vardenafil and at least 48 hours after tadalafil. The prescriber should choose another treatment.
