Ten minutes after an intravenous antibiotic is started, a patient develops wheeze, facial swelling and a blood pressure of 78/40 mmHg. The doctor is not on the ward. The hospital's anaphylaxis PPPG authorises trained nurses to give intramuscular adrenaline. The nurse has stopped the infusion and called for help. What should she do next?

  1. AWait for the doctor to arrive and prescribe adrenaline before giving anything
  2. BGive an oral antihistamine, which nurses may give without a prescription in an emergency
  3. CGive intramuscular adrenaline per the local PPPG, within her competence, and document it
  4. DRestart the antibiotic at a slower rate to see whether the symptoms return

Answer: C. NMBI's 2020 guidance allows a nurse to give a non-prescribed medication when it is needed to reduce a threat to the patient's life and no prescriber is immediately available, provided she acts within her scope and follows local PPPGs. This patient has signs of anaphylaxis with shock, and intramuscular adrenaline under the local protocol is the life-saving step.
